Collection of Personal Information

 

Registration Forms

When any student joins a Yoga for You class, they are asked to complete a registration form.  This includes a Health and Safety Questionnaire and Opt-in to consent to receive marketing emails and / or emails / texts / phone calls to advise of last minute changes to classes eg cancellations.  Record keeping of students and their attendance is a requirement of my insurance company.

 

Health Status Questionnaires

These assist Yoga for You in planning classes and to help students get the most out of each class. The forms ask for name, contact details and information about health status of the individual joining the class and an emergency contact plus a date and signature to say that the information given is correct.

 

The information given by students will only be shared with a 3rd party in the case of class cover.  The 3rd party will be a qualified and insured Dru Yoga teacher and be approved by Yoga for You.

 

In the event of an emergency, personal information and / or emergency contact details may be shared with the emergency services.

 

Consent to Contact

Email Contact

Students are given the option to receive by email a Yoga for You newsletter containing information about classes and / or workshops that may be of interest to you and articles or videos about yoga and well-being subjects.  Opted-in email addresses will be added to a database so that emails may be sent to you.

 

You may change your preference regarding this by emailing info@yogafor-you.co.uk or following the Unsubcribe instruction in each email newsletter.

 

Students are also given the option to be notified by email in the event of last minute information, such as cancellation of a class.

 

Text / Telephone Contact

Your consent will be requested to add your phone number to my phone which is password protected. You will only be contacted by text or by phone call in the event of last minute information, such as cancellation of a class or to reply to a message or phone call from you.

 

Emergency Contact information is collected solely for use in an emergency, should Yoga for You need to contact your next of kin or give this information to the emergency services.

 

Storage of Student Registration Forms

These forms are stored in a locked metal box file at my home, when they are not in use at class.  I am the only key holder to the box and nobody else has access to it. 

 

Any sensitive information collected via email will be transferred to a paper copy and dealt with as above. The email will then be deleted.

 

Information is stored for 7 years after your last class to comply with insurance company criteria.  It will then be destroyed by shredding.

 

Electronic record keeping

All student questionnaire records are kept in hard copy format ie on paper.

 

I keep an electronic database of student contact details to record date of attendance at class.  A record of attendance is required by my insurance company and to determine the 7 year non-attendance period at which point electronic records are deleted and paper records are destroyed by shredding. 

 

This data will also be flagged to show whether consent has been given to email the Yoga for You newsletter.  Where consent is given, the data will be used to email the Yoga for You newsletter.

 

This database will also be flagged to show whether consent has been given for contact by email, text or telephone to be notified of last minute changes to classes.

 

If students enquire by email and I reply by email, these emails are kept only as long as necessary, details are not added to a database for newsletter purposes unless consent has been given.

 

Registers containing student names will also be held as paper copies and may also contain contact details for notification of last minute changes such as cancellation of classes as well as emergency contact information.

Service User Access to data

In line with ICO guidelines, I will respond to subject access requests for any data I hold.  Following guidelines, requests need to be made in writing and a response will be given within the 1 month allowed. There will be no charge made for data requests. I reserve the right to refuse or charge for requests that are manifestly unfounded or excessive, informing the individual why and notifying the relevant supervisory authority within the month.
